New Streaming Server

Given a streaming service, add a new streaming server to MixEffect.

Name: The name of the streaming server. e.g. Primary
URL: The url for the streaming server.
Service: The streaming server that the server belongs to.
Stream Key: The stream key.
Set Default Server: Toggle to set this server as the default for the streaming service.
